I/We understand and appreciate that participation or observation of the sport constitutes a risk to the participant of serious injury, including permanent paralysis or death. I/We voluntarily and knowingly recognize, accept and assume this risk and release Innovative Hockey Tutoring, the hockey school Directors/ Instructors/  Staff  and  the  Arena/Facility  from  any  and  all  claims.  I/We  shall  not  hold anyone  responsible  for  any  accident  or  injury  to  participant(s)  or  observer(s), damage  or  loss  of  personal property,  however  caused,  whether  on  or  off  the  premises.  In  signing  this  application,  the participant,  the  parent  or  guardian certifies that the applicant/participant is in good physical and mental health. In the event of emergency, I/we hereby give our permission to the school per-sonnel to administer/seek any medical attention as they feel necessary for my/our child.I acknowledge that we have read and understood the terms and conditions of this application and agree to abide by the terms and conditions.
